精神分裂症患者及一级家属近红外光谱成像的空间特点

摘要:
目的 探讨精神分裂症患者及一级家属近红外光谱成像( NIRS)检测的空间特点。方法 入组符合入组标准的精神分裂症患者、一级家属、健康人共三组各 20例,结合言语流畅性测试(VFT)行NIRS检测,分析3组的氧合血红蛋白(Oxy-Hb)脱氧血红蛋白(Deoxy-Hb)和总氧血红蛋白(Total Hb)的各通道的变化规律。结果 3组比较差异具有统计、学意义的有 Oxy-Hb的通道 6,7,11,14,15, Deoxy-Hb的通道11,18,20,Total Hb的通道6,7,11,13,14,15。Oxy-Hb及Total Hb通道6,7,11,14, 15健康对照组均为负值,而患者组均为正值。通道 6,7的Oxy-Hb、Total Hb相对平均值,健康对照组低于患者组及家属组,通道18,20的Deoxy-Hb相对平均值,患者组、家属组低于健康对照组;通道 14, 15的Oxy-Hb、Total Hb相对平均值及通道 11的Deoxy-Hb相对平均值,患者组高于健康组及家属组。结论 患者组及家属组前额叶功能存在异常:右中背侧区的异常激活、左前额叶腹侧区的血流减慢可能为状态学标记;前额叶的左中腹侧区激活增强及右侧腹外侧区血流减慢不足可视为精神分裂症候选的生物学标记。

Spatial characteristics of near infrared spectroscopy in schizophrenia patients and their first degree rel-atives

Abstract:
【Abstract】 Objective To investigate the spatial characteristics in schizophrenia patients and their first degree relatives by Near Infrared Spectroscopy(NIRS). Methods Subjects who met the inclusion crite-ria were divided into three groups:the schizophrenia patients (group A,n=20),first degree relatives (group B, n=20)and heathy persons (group C,n=20). NIRS combined with Verbal Fluency Test were tested. The changes of oxyhemoglobin (Oxy-Hb),deoxy hemoglobin (Deoxy-Hb)and total hemoglobin (Total Hb)in three groups were analysed. Results Significant differences were found in Oxy-Hb (channel 6,7,11,14,15),Deoxy-Hb (channel 11,18,20)and Total Hb (channel 6,7,11,13,14,15)in three groups. Oxy-Hb and Total Hb(channel 6,7,11,14,15)were negative in group C,while were positive in group A. Relative average values of Oxy-Hb and Total Hb (channel 6,7)in group C were lower than that in group A and B. Relative average values of Deoxy-Hb (channel 18,20)in group A and B were lower than that in group C. Relative average values of Oxy-Hb and Total Hb(channel 14,15)and Deoxy-Hb(channel 11)in group A were higher than that in group B and C.Conclusions The prefrontal cortex function of schizophrenia patients and their first degree relatives is abnor-mal. The abnormal activation of dorsal area in the right and slow blood flow of ventral area in left prefrontal lobe may be state markers. The enhanced activation in left ventral prefrontal cortex and blood flow deficiency in right ventrolateral prefrontal cortex can be regarded as candidate biological markers of schizophrenia.
